An Overview of the Case
A 32-year-old male presented to Kaizen Fistula Care Clinic for treatment of a complex fistula. He reported experiencing perianal pain and pus discharge, which made it difficult for him to sit comfortably.
The patient had a history of three prior surgeries in the UK:
- First surgery: Perianal abscess drainage
- Second surgery: Seton therapy
- Third surgery: Endorectal flap surgery
Despite these interventions, his fistula persisted, and he continued to experience significant pain. Two additional surgeries in India were also unsuccessful. Finally, his surgeon referred him to Kaizen Fistula Care for further management.
Upon arrival at Kaizen Fistula Care, the patient reported significant perianal pain. To better understand the anatomy of the fistula, Dr. Samrat Jankar performed an MRI scan. An anorectal manometry was also conducted, revealing decreased perianal tone likely due to the multiple previous surgeries.
Following a comprehensive evaluation, sphincter-preserving surgery was performed. This involved a fistulotomy with primary sphincter repair.
The patient was discharged with a draining wound. As he was from another state, he booked accommodation near the clinic. During the first 15 days, he visited the clinic twice a day to utilize the available wound care services. Due to pain and other issues, dressing changes took 15-20 minutes each time. However, after one month, the patient achieved complete healing and reported a complete resolution of his symptoms, including pain. Following his recovery, he was able to return to his professional duties in the UK.

Case About the Fistula
.
The patient presented with a perianal infection that progressed to complete perianal sepsis. Due to the severity and being outside of India, he underwent perianal abscess drainage. However, the size of the abscess necessitated a colostomy for better wound management.
Once back in India, his colostomy was reversed. Unfortunately, he developed a large ischiorectal and perirectal abscess following the reversal.
Upon arrival at Kaizen Gastro Care, a thorough evaluation revealed a complex fistula involving the ischiorectal and perirectal areas, along with a supralevator abscess. This presented a significant challenge.
A single-stage surgical procedure (PILTEC) was performed to address the anal opening and simultaneously drain the bilateral ischiorectal and supralevator abscesses.
Following surgery, a drainage tube was used for the initial 10 days to facilitate dressing changes and ensure proper healing. As of now, approximately 45 days post-surgery, the wound, fistula, and abscess have all completely healed.
The patient has made a full recovery from the fistula.
